Description of Incident: On Tuesday October 16th at approx. 315pm while there was there was an NOI special call out going on from 1 to 630 pm. The facilitator exited the Chapel through 2 wooden doors, one was bolted closed, and entered the Chapel Officers area. The facilitator was agitated over an issue. He approached the Officer’s desk where the area Sgt. was. Inmate stated that he would take over area if situation was not resolved. He also stated that he already gave the order to rest of the 46 inmates still inside Chapel. He also stated the order not to move anywhere was given. The Sgt. immediately approached the Chapel door. At this time the facilitator attempted to gain access to Chapel and was told by me to step back. He again attempted to enter Chapel and was told again to stay back. At this time the facilitator hit me in the upper back area and pushed me into the sergeant in an attempt to enter Chapel. Upper body holds were used to attempt to gain control of facilitator who was violently fighting. At this point approximately 10-15 inmates rushed the door and tried to open door. Three Officers attempted to close the door and the inmates continued to push. A response was called and more Officer’s came to hold the door closed, while still attempting to gain control of violently fighting/ struggling facilitator. As the response came to the Chapel Officers area, the inmates inside Chapel backed off the door and control was gained of facilitator. 6 officers ultimately went to the local Emergency Room with injuries and only 1 returned to work.
