PT was verbally redirected multiple times for speaking during medication pass when he became agitated on the ward. SHTA staff redirected the Pt. and offered quiet room, but the Pt declined. Pt continued aggressive and threatening behavior during transit of ward occupants to program area in TAC building. SHTA staff again attempted verbal de-escalation techniques and redirected Pt to the available quiet room where he turned and struck the SHTA in the left side of the neck. A brief manual restraint was implemented and patient continued to be extremely difficult during the restraint process, struggling with SHTA staff and presenting imminent danger to himself and others. SHTA also sustained injuries to his wrists and elbow during the restraint process.
